Where did you see him/her? Please be as specific as possible about the location - including city, state or address, if you can.
When did you last see this person?
Provide as much of the following as possible based on the person you saw:
Additional physical description (such as hair style, facial hair, scars, tattoos, what they wearing
or other distinguishing physical features)
Was he/she using a car/vehicle? If so, vehicle's make/model? Color? Approximate year? License plate?
Any other comments about what you saw:
On scale from 1 - 10 (10 being the highest) how sure are you that this is the person we're looking for?
Your tip might provide valuable insight and investigators may want to ask you for more information. Additionally, some cases provide rewards for information leading to capture or recovery.
